A Call To Care, Inc. d/b/a Mission of Mercy Pittsburgh, (“MOMPgh”) is committed to protecting the privacy of our donors, volunteers, patients, and the personal information you may share with MOMPgh or that MOMPgh holds. MOMPgh values our your trust and recognizes that maintaining this trust requires the charity to be transparent and accountable to our supporters and the public at all times. This Privacy Policy (the “Policy”) describes the information collected about our donors, volunteers, website users, and patients (“you”), and MOMPgh’s practices for using, maintaining, protecting, and disclosing that information.
1. Information Collected and Maintained. MOMPgh collects and maintains personal information that you provide to us when you:
- Contact us by phone, mail, or email, in person, or via our website.
- Register for an event.
- Make a contribution, either on our website, at a special event, over the phone, or by mailing in a check.
- Navigate through our website.
- Attend an event.
From time to time, we may collect personal information from you in ways not described above, subject to your express approval.
2. Types of Information. MOMPgh collects and maintains the following types of information about:
- Donor Personal information, including:
- Contact information, such as name, address, email, and telephone number.
- Giving information, including types and amounts of donations and credit card information. We utilize secure transaction methods when collecting credit card information over the internet. MOMPgh does not disclose credit card account information provided by our donors, except for the proper processing of donations.
- Information on events attended and publications received.
- Any other information provided by the donor to MOMPgh, such as public comments or posts on MOMPgh’s website.
- Volunteer Personal information, including:
- Contact information, such as name, address, email, telephone number, username/password (used to recall your information and preferences when you visit the Website to make changes and/or select additional volunteer opportunities).
- Professional certification information, if applicable to your volunteer type.
- Emergency Contact Information (Name, Relationship and telephone number) utilized only in the event you experience an emergency at an event.
- Any other information provided by the volunteer to MOMPgh, such as public comments or posts regarding MOMPgh or optional profile photos uploaded to the Website’s volunteer registration page, or information provided with your explicit consent to-be used for advertising or promoting MOMPgh (specifically any pictures, voice recordings, or reproductions of physical likeness).
- Patient Personal information (collected at events only and not through the Website) including:
- Contact information, such as name, address, email, telephone number.
- Demographic information, such as ethnicity, veteran status, gender, health history, allergies or medical complaints.
- Emergency Contact Information (Name, Relationship and telephone number) utilized only in the event you experience an emergency at an event.
- Any other information provided by the patient to MOMPgh at the event. MOMPgh will request only the information necessary to adequately assess a patient’s situation and to develop and implement effective service and/or treatment plans, including delivery of services and appropriate referrals. MOMPgh will not take identifiable photographs, film, or otherwise record patients without their informed, written consent for purposes other than identification, diagnosis, or as required by law. MOMPgh will make every effort to ensure confidentiality of information about patients and will inform patients of the conditions under which information may be requested, the possible consequences of the patient’s disclosure of information, and the conditions under which the organization may be legally or ethically required to release confidential or private information. MOMPgh will comply with all Federal, State, and local regulations relating to the protection of confidential information including, but not limited to, Protected Health Information (PHI). We only collect sensitive personal information if we have a legitimate business need to facilitate your treatment at an event, and we keep that information only for as long as it is necessary to fulfill the purpose stated for its collection.
- Non-personal information that does not personally identify you, including information that is anonymized, aggregated, or publicly available when not combined with non-publicly available information about you. MOMPgh will not discuss any information about prospective or current donors and volunteers outside of the work environment without first anonymizing or aggregating it. We will not lease, sell, rent, or exchange any personal information collected to other organizations or individuals.
- Information about your equipment, browsing actions, and patterns, including information automatically collected through technical methods such as:
- Cookies (or browser cookies). A cookie is a small file placed on the hard drive of your computer. You may refuse to accept browser cookies by activating the appropriate setting on your browser. However, if you select this setting you may be unable to access certain parts of our website. Unless you have adjusted your browser setting so that it will refuse cookies, our system will issue cookies when you direct your browser to our website.
- Flash Cookies. Certain features of our website may use local stored objects to collect and store information about your preferences and navigation to, from, and on our website. Flash cookies are not managed by the same browser settings as are used for browser cookies.
- Web Beacons. Pages of our Website [and our e-mails] may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit MOMPgh, for example, to count users who have visited those pages or [opened an email] and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity).
3. Confidentiality. All information about any financial and non-financial transactions between donors and MOMPgh is considered highly confidential and we employ appropriate security measures designed to protect it. While the information is kept confidential, it is shared with MOMPgh’s board members, staff, and professionals and with any of MOMPgh’s parent organizations or coordinating sponsors on a need-to-know basis and in compliance with all laws, regulations, court orders, or other governmental requests. All information about any volunteers or patients are similarly considered highly confidential, are appropriately secured and disposed of after they are no longer needed.
4. Use of Personal Information. MOMPgh collects and maintains personal information to:
- Establish a relationship and communicate with you about MOMPgh, including our programs, special events, and funding needs.
- Learn about our donors and volunteers and what matters to you.
- Process donations and event registrations and issue tax receipts.
- Carry out our obligations arising from your donation, registration as a volunteer, or attendance at an event as a patient, so that we comply with all laws and regulations, including reporting requirements or to comply with any court order, law, or legal process, including to respond to any government or regulatory request.
- Plan future fundraising activities and events.
- Analyze giving patterns.
- Solicit feedback from you by conducting surveys.
- Fulfill the purpose for which you provide it. For example, to provide treatment to patients as-needed or verify a volunteer’s qualifications to provide treatment to patients.
- Fulfil any other purpose disclosed by us when you provide the information to us.
- Protect the rights, property, or safety of our donors, volunteers, patients, or others (for example, for the purposes of fraud protection and credit risk reduction).
MOMPgh uses industry standard safeguards to protect your personal information against the unauthorized access, use, alteration, or destruction of your personal information. MOMPgh will not share, sell, or exchange your personal information with any other entity for third-party fundraising or marketing purposes, except as provided in this Policy or as expressly consented to by volunteers upon registering to attend an event. To opt out, you may contact MOMPgh as described in the “Contact Us” section below. To the extent that a third-party service provider is used to process any donations, the provider is bound by strict confidentiality rules, and your information will only be used to the extent necessary to process the donation. The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Website, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our Website. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Website.
5. Communications. MOMPgh may from time to time communicate with donors or volunteers, including by telephone, text, email, and mail, concerning MOMPgh’s charitable activities, events, and requests for future support. MOMPgh will not call or email you or send mailings to you on behalf of other organizations. To opt out of receiving these communications, you may contact MOMPgh as described in the “Contact Us” section below or by clicking on the “unsubscribe” link in any email communications. MOMPgh will not use personal information to communicate with patients for purposes other than to facilitate their treatment at an event.
6. Records Retention and Destruction. MOMPgh complies with all Internal Revenue Service, state, and local regulations and MOMPgh’s internal policies governing the retention, management, and destruction of donor information collected and maintained by MOMPgh. We generally retain Personal Information only for so long as it may be relevant for the purposes identified in this Privacy Policy, unless a longer period of time is required by law, and we dispose of information that is no longer needed to fulfill the purpose stated at the time of its collection or in this Privacy Policy.
7. Accessing and Correcting Your Personal Information. If MOMPgh still has relevant personal information, you can review and request changes to the personal information that MOMPgh has collected about you by contacting MOMPgh as described in the “Contact Us” section below. However, please be aware that MOMPgh may not accommodate a change request that would impact record retention or other operational requirements like remembering email opt-out preferences, violate any law or legal requirement, or cause the information to be incorrect. Deleting your personal information may also require deleting your user account (if any).
8. Changes to the Policy. This Policy is current as of the date stated at the top of this Policy. MOMPgh reserves the right to amend this Policy from time to time. MOMPgh will notify you of any material changes to the Policy through a notice on the Website home page. You are responsible for ensuring we have an up-to-date active and deliverable email address for you, and for periodically visiting our Website and this privacy policy to check for any changes.
9. Children Under the Age of 13. Our Website is not intended for children under 13 years of age. No one under age 13 may provide any information to the Website. We do not knowingly collect personal information from children under 13. If you are under 13, do not use or provide any information on this Website or provide any information about yourself to us, including your name, address, telephone number, email address, or any user name you may use. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us. Any patients attending an event under the age of 18 must be accompanied by a legal guardian until all treatment is complete. If a guardian needs treatment for themselves, there must be an additional adult present with his/her children while the guardian is being treated. The guardian cannot be treated the same day as their children unless there is another adult to watch the children. Any volunteers registering for an event must be at least 16 years of age when volunteering.
10. Donor Bill of Rights. MOMPgh’s donor practices align with the Donor Bill of Rights as created by the Association of Fundraising Professionals, Associations for Healthcare Philanthropy, the Council for Advancement and Support of Education, and the Giving Institute. A copy of the Donor Bill of Rights may be available by contacting MOMPgh as described in the “Contact Us” section below.
